free alternative to Google Forms?

Hi,
Google Forms is a survey administration program in Google Drive. It lets me ask questions, and peoples' responses can go to a spreadsheet in Google Drive. Do you know of such a program that I can self-host? Is it ethical to collect information in this way (not the collection itself, but the SaSS approach instead of offering the survey as free software)?

https://framaforms.org allows to easily create simple forms but looks quite hard to setup: https://framagit.org/framasoft/framaforms
